This shows that the problem was indeed related to permissions. Set permissions (eg, for Everyone) on other files apart from the MAP file.

Did you create the "shp" folder under /Downloads/Web as explained in the readme? Can the user account impersonating web connections (normally, IUSR_...) access that folder?
